Temporary Handicapped Placard Please completed this form along with your doctor and return it to Police Headquarters with a $4.00 check made payable to “NJ MVC.” It will be reviewed by the Chief and you will be contacted when it is complete.
If you are looking for a permanent placard or disabled license plates please refer to the New Jersey Department of Motor Vehicle Commission for instructions and applications.
Permit To Solicit All forms must be filled out and returned to Det. Kucinski in order to start the process to be approved for Soliciting Door to Door within the Borough.

Bicycle Registration The Spring Lake Police Department maintains a record of resident’s bicycles in the event that they are lost or stolen. Information will be maintained and used to enter the bicycle as stolen or return the bicycle when it is located. Complete the form and return it to Spring Lake Police Headquarters to received a registration sticker to apply to your bicycle. Click here for the form. House Check If you are going on vacation, or planning to be away from your home for an extended period of time, fill this form out to make the department aware of important emergency information regarding your house.
Personal Emergency Sheet This document, designed by a Spring Lake high school student, is meant to be displayed prominently in the home for police or medical personnel to see, in case of emergency. Sheet contains family/friend contact information, medical conditions, doctor names and phone #’s, medications, allergies, etc.
